Job Responsibilities

Provide professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment.
Conduct assessments and planning for patient care.
Coordinate care and manage resource utilization.
Ensure discharge planning is implemented effectively.
Document interventions and maintain regulatory compliance.
Advocate for patients' needs.

Requirements

Experience: 2 years of Acute care experience in a Medical Surgical or Inpatient setting

Education: Associate's Degree - Nursing degree/diploma upon hire.

License & Certification:

Alaska Registered Nurse License upon hire
