Job Responsibilities
- Support in-home, patient-facing visits to address patient care needs.
- Perform primary care in-home services and initial patient screenings/physical examinations.
- Create appropriate assessment/plans and complete documentation in a timely manner.
- Partner with other clinicians in the Care Community to manage patient health and wellbeing.
- Engage attributed patients into the WellBe program and support care needs.
- Facilitate decision-making by providing educational resources to staff, patients, and families.
- Address any urgent care needs of the patients and conduct transition of care visits.
- Collaborate with the community team to complete a full patient care assessment and plan.
- Order, interpret, and analyze diagnostic tests; determine follow-up appointments and treatment options.
- Prescribe and manage medications and treatments as appropriate.
- Update patient records and ensure accuracy at each patient appointment.
- Deliver patient care and complete regular patient reviews with supervising physician as per state supervisory agreement.
- Focus clinical care delivery on documentation/coding, HEDIS measures, patient engagement, and managing medical expenses.
- Utilize available resources to meet patient needs and refer to internal/external resources as appropriate.
- Collaborate with the interdisciplinary team and participate in scheduled team meetings.
- Perform other tasks as needed to accomplish team's objectives.
Required Skills
Core Skills
- Master's Degree in Nursing with a focus on Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner or Adult-Geriatric Nurse Practitioner
- Active Nurse Practitioner license in the state of job posting
- Current board certification with ANCC or AANP
- BLS certification and valid NPI number
- CPR certification
- Active DEA licensure or eligibility to obtain
- Valid driver's license and access to a car
Preferred Skills
- 3+ years of Nurse Practitioner experience
- Prior healthcare experience in medical assistant, certified nursing assistant, EMT/paramedic, lab assistant, or registered nurse roles
- Experience in value-based care, home care, primary care, hospice, palliative care, or geriatrics
- Experience with telehealth care delivery
- Proficiency with Microsoft Office and Electronic Health Records
Requirements
Experience: 3+ years Nurse Practitioner experience preferred; will consider new grads
Education: Master's Degree in Nursing with focus on Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner or Adult-Geriatric Nurse Practitioner
License & Certification:
- Active Nurse Practitioner license
- Current board certification with ANCC or AANP
