Job Responsibilities
- Present VITAS services and philosophy to patients and families to facilitate admissions.
- Participate in information gathering, analysis, education, and form completion for admissions.
- Evaluate patients for hospice appropriateness through medical records review.
- Obtain agreement for the admission of referred patients and families who meet criteria.
- Analyze data collected by LPN to facilitate or obtain admission orders.
- Build effective referral source relationships to uphold quality and responsiveness.
- Maintain clinical and professional competence.
- Develop effective internal relationships with admissions and patient care resources.
- Utilize interpersonal skills to educate and address questions from patients and families regarding hospice and palliative care.
Required Skills
Core Skills
- Knowledge of intake, admission, and transition to care processes.
- Effective communication skills with patients, families, physicians, and colleagues.
- Clinical knowledge of disease manifestations.
- Ability to use VITAS information system and mobile technology platforms.
- Reliable transportation.
Preferred Skills
- Hospital, physician practice, LTC, or ALF experience.
- Strong presentation skills.
- Previous sales or service experience.
Requirements
Experience:
Education: Bachelor’s degree preferred.
License & Certification:
- Currently licensed as a RN in the state of duties performed.
