Completes initial in-person visits for patients enrolled into Wellstar MCG Health's Virtual Care @ Home program.
Conducts initial assessments of patients and families to coordinate care plans, treatment goals, and ancillary needs.
Actively identifies and enrolls patients from AUMC Inpatient, ER, or clinic settings into Virtual Care @ Home programs.
Performs clinical assessments based on patient condition and discharge goals, coordinating expected treatment plans with Virtual Care Physician.
Educates patients and family/caregivers on the Virtual Care @ Home program and goals, self-care techniques, medications, and disease management.
Identifies social barriers or needs that impact patient outcomes and satisfaction of the program.
Seeks opportunities to improve the Virtual Care @ Home Admissions and Enrollment process.
Coordinates enrollments and patient consents with the Virtual Care @ Home Admitting Specialist.
Works with the Virtual Care @ Home Social Worker, Inpatient Social Worker, and Inpatient Care team on timely discharges.

Experience: Minimum 3 years acute care nursing for ICU or medical-surgical units required or Minimum 5 years Home Health or Hospice preferred.

Education: Associates Nursing or Diploma Nursing required; Bachelor's Nursing preferred.

License & Certification:

ACLS-P - ACLS - Provisional within 180 Days or ACLS-I - ACLS - Instructor or ACLS - Advanced Cardiac Life Support
BLS-I - Basic Life Support - Instructor or BLS - Basic Life Support
RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
