Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts systematic, dynamic assessment of patients including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
- Establishes daily goals with the patient and family.
- Formulates a plan of care and transitions or discharges planning.
- Implements nursing care according to the care plan to assure continuity.
- Documents patient care in the healthcare record.
- Evaluates patient's status and the effectiveness of nursing care, modifying care as needed.
- Coordinates care by managing and correcting the sequence, timing, and effectiveness of patient care.
