Job Responsibilities
- Report directly to the AVP of Business Development.
- Ensure the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
- Complete the admissions process on referred patients in emergency cases as staffing indicates and for inpatient hospice admissions.
- Provide accurate and timely information regarding hospice services to anyone inquiring.
- Receive calls from referral sources, respond timely, and initiate paperwork to begin the Intake/referral process.
- Serve as a patient advocate in evaluating/developing the plan of care and obtaining informed consent for hospice.
- Call on multiple account types, including SNFs, ALFs, Hospitals, and Physicians.
- Offer in-services and education on the benefits of hospice.
Required Skills
Core Skills
- Compassionate, kind and empathetic demeanor
- RN license in the state(s) of employment
- Healthcare experience
Preferred Skills
- 2 years of healthcare experience in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
- Being known and respected within your community.
