Job Responsibilities
- Respond to clinical referral information with care and expertise.
- Address inquiries and requests with a compassionate touch.
- Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
- Coordinate care to ensure patient comfort and quality of life.
- Deliver top-notch nursing care rooted in proven principles and techniques.
- Educate and empower patients and families in creating personalized care plans.
- Participate in call duties as part of the team.
Required Skills
Core Skills
- Current unencumbered license as a Registered Nurse
- Minimum one year experience as an RN in a medical surgical/acute care setting
- Computer proficiency including EMR background
- Current CPR certification (if required by state)
Preferred Skills
- Experience in hospice care
