Job Responsibilities
- Ensure patients receive quality and comprehensive coordinated care services.
- Promote timely access to appropriate care and decrease emergency room visits.
- Increase utilization of preventative care services and promote patients’ self-management.
- Act as an advocate, interpreter, educator, and counselor for oncology patients and their families.
- Create and promote adherence to treatment and survivorship care plans in coordination with patients and multidisciplinary teams.
- Serve as a primary contact point, advocate, and informational resource for patients and families.
- Demonstrate knowledge and coordination to provide care from screening to survivorship.
- Develop and manage care plans with patients and families, including treatment and survivorship plans post-treatment.
- Monitor adherence to care plans and evaluate effectiveness in a timely manner.
- Facilitate communication to support patient and family caregiver requests for care coordination.
- Systematically assess, plan, implement, and evaluate oncology patient care.
- Provide education and information to patients and families for seamless care.
- Initiate and coordinate chemotherapy education appointments prior to treatment initiation.
- Document patient teaching and respond to requests for information regarding disease processes.
- Support families through decision-making periods and coordinate emotional support activities.
- Cultivate collaboration with primary and specialty care providers through communication and follow-ups.
- Identify high-risk patients and assist in overcoming barriers to care.
- Coordinate patient tests, procedures, appointments, and treatments across healthcare disciplines.
- Refer patients to supportive care services such as nutritionists and social workers.
- Assist patients in clinical trial coordination.
- Participate in multidisciplinary consultations and serve as liaison between patients and medical staff.
- Maintain a regular attendance and fulfill overtime as needed.

Requirements
Experience: Five years of professional nursing experience, three of which were in oncology OR seven years of professional nursing experience, five of which were in oncology.
Education: Bachelor’s degree in nursing from an accredited institution or graduation from an accredited school of nursing meeting Alabama state standards.
License & Certification:
- Current licensure as a registered nurse in Alabama
- Certification to administer chemotherapy
- ACLS certification (to be acquired within six months of employment)
