Job Responsibilities

Ensure patients receive quality and comprehensive coordinated care services.
Promote timely access to appropriate care and decrease emergency room visits.
Increase utilization of preventative care services and promote patients’ self-management.
Act as an advocate, interpreter, educator, and counselor for oncology patients and their families.
Create and promote adherence to treatment and survivorship care plans in coordination with patients and multidisciplinary teams.
Serve as a primary contact point, advocate, and informational resource for patients and families.
Demonstrate knowledge and coordination to provide care from screening to survivorship.
Develop and manage care plans with patients and families, including treatment and survivorship plans post-treatment.
Monitor adherence to care plans and evaluate effectiveness in a timely manner.
Facilitate communication to support patient and family caregiver requests for care coordination.
Systematically assess, plan, implement, and evaluate oncology patient care.
Provide education and information to patients and families for seamless care.
Initiate and coordinate chemotherapy education appointments prior to treatment initiation.
Document patient teaching and respond to requests for information regarding disease processes.
Support families through decision-making periods and coordinate emotional support activities.
Cultivate collaboration with primary and specialty care providers through communication and follow-ups.
Identify high-risk patients and assist in overcoming barriers to care.
Coordinate patient tests, procedures, appointments, and treatments across healthcare disciplines.
Refer patients to supportive care services such as nutritionists and social workers.
Assist patients in clinical trial coordination.
Participate in multidisciplinary consultations and serve as liaison between patients and medical staff.
Maintain a regular attendance and fulfill overtime as needed.

Requirements

Experience: Five years of professional nursing experience, three of which were in oncology OR seven years of professional nursing experience, five of which were in oncology.

Education: Bachelor’s degree in nursing from an accredited institution or graduation from an accredited school of nursing meeting Alabama state standards.

License & Certification:

Current licensure as a registered nurse in Alabama
Certification to administer chemotherapy
ACLS certification (to be acquired within six months of employment)
