Job Responsibilities
- Ensure patients at Mitchell Cancer Institute receive quality and comprehensive coordinated care services.
- Promote timely access to appropriate care and decrease emergency room visits.
- Increase utilization of preventative care services and promote patients’ self-management and shared decision making capabilities.
- Represent the multidisciplinary team of specialists as an advocate, interpreter, educator and counselor for oncology patients and their families.
- Create and promote adherence to treatment and survivorship care plans developed in coordination with patients, families and the multidisciplinary team.
- Serve as primary contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources.
- Demonstrate knowledge, skill, and coordination to provide care and guidance to cancer patients from screening to survivorship.
- Work with patients to plan and monitor care, including unmet health and social needs.
- Develop care plans with patients and families, providing treatment and survivorship plans post-treatment.
- Monitor adherence to care plans and evaluate effectiveness, facilitating care changes as needed.
- Systematically assess, plan, implement, and evaluate the care of oncology patients.
- Provide education and information to patients and families, ensuring seamless and comprehensive care.
- Initiate appointments for chemotherapy education and coordinate completion before chemotherapy initiation.
- Document patient teaching and respond to requests for information regarding disease processes and community resources.
- Support families through difficult decision-making periods and assist in care coordination.
- Cultivate primary care and specialty provider co-management with timely communication.
- Assist in identifying high-risk patients and coordinate patient tests, procedures, appointments, and treatments.
- Help patients overcome barriers to care including transportation and financial counseling.
- Refer patients for supportive care services including nutritionists and social workers.
- Assist eligible patients with clinical trial coordination.
- Participate in multidisciplinary consultations and serve as a liaison between patients and medical staff.
- Maintain regular attendance and work schedule as defined, including overtime as required.

Requirements
Experience: Five years of professional nursing experience, three of which were in oncology, or seven years of professional nursing experience, five of which were in oncology.
Education: Bachelor’s degree in nursing from an accredited institution.
License & Certification:
- Current registered nurse licensure in Alabama
- Certification to administer chemotherapy within six months of employment
- ACLS certification within six months of employment
