Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
Identifies problems and sets measurable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family and formulates a plan of care.
Implements nursing care according to the care plan.
Ensures patient safety and administers medications.
Provides education to patients and families.
Documents care in the healthcare record.
Continuously evaluates the patient’s status and the effectiveness of nursing care.
Manages and coordinates patient care from admission to discharge.
Delegates tasks and supervises nursing staff as needed.
Prioritizes care based on patient needs.
