Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Uses a systematic, dynamic way to collect and analyze data about a patient.
- Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
- Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care and documents assessment data and goals in the patient's care plan.
- Implements nursing care according to the care plan, ensuring continuity of care.
- Documents care in the patient's record.
- Ensures patient safety and administers medication as ordered.
- Provides patient/family education.
- Continuously evaluates patient status and the effectiveness of nursing care.
- Initiates care coordination actions with patients, families, and/or healthcare team members.
- Demonstrates delegation, supervision, and prioritization in patient care.
Required Skills
Core Skills
- Valid Arizona Nursing License or compact state license
- Basic Life Support (BLS) certification
- Patient assessment and care planning skills
Preferred Skills
- Advanced Cardiac Life Support (ACLS) certification
- Pediatric Advanced Life Support (PALS) certification
- Neonatal Resuscitation Program (NRP) certification
Requirements
Experience: None
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid Arizona Nursing License or compact state license
- Basic Life Support (BLS) certification
