Job Responsibilities
- Act as a Skilled Nursing Facility’s leading provider of internal medicine services.
- Collaborate with the facility’s interdisciplinary team and physicians.
- Serve as a day-to-day clinical leader while overseeing the care of all residents.
- Evaluate new-admission visits.
- Coordinate discharge visits.
- Conduct monthly stability visits for long-term residents.
- Evaluate patients exhibiting changes in condition.
- Provide advanced care planning.
- Perform comprehensive health assessments, differential diagnosis, and prescribe treatments.
- Prescribe medications and treatments under protocol for the management of acute and chronic illness.
- Perform comprehensive physical examinations of patients.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.
Required Skills
Core Skills
- Advanced Practice Licensure (APN) with a Florida license
- Active license and DEA number
- Ability to build rapport with patients and their loved ones
- Thriving in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience
Preferred Skills
- Previous experience in nursing or other medical fields
