Job Responsibilities
- Act as the Skilled Nursing Facility’s leading provider of internal medicine services
- Collaborate with the facility’s interdisciplinary team and physicians
- Serve as a day-to-day clinical leader while overseeing the care of all facility residents
- Work with the facility’s patient population, including short and long-term residents
- Oversee day-to-day care with the assistance of the facility’s nursing staff
- Provide guidance from the facility’s medical directors and attending physicians
- Evaluate new-admission visits
- Coordinate discharge visits
- Conduct monthly stability visits for long-term residents
- Perform evaluations for patients exhibiting changes in condition
- Provide advanced care planning
- Conduct comprehensive health assessments, differential diagnosis, and prescribe treatments
- Prescribe necessary medications and treatments
- Perform comprehensive physical examinations of patients
- Develop and implement patient management policies and procedures
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with attending physicians and/or specialists regarding patient care
Required Skills
Core Skills
- Advanced Practice Licensure (APN) with a Florida license
- Active license and DEA number
- Ability to build rapport with patients and their loved ones
- Ability to thrive in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
Preferred Skills
- Previous experience in nursing or other medical fields
- Master’s degree required
