- Conduct comprehensive Health Risk Assessments for Medicare Advantage patients.
- Review patients' medical history to gain insights into their health.
- Understand patients' social environment to enhance care quality.
- Identify and bridge multiple care gaps in one visit.

- Board-certified Nurse Practitioner
- 1+ years of Nurse Practitioner experience
- Home health experience
