Job Title: Nurse Practitioner (CLC)
Position: Key clinical leader and member of the Interdisciplinary Treatment Team
Job Type: Full Time

Job Responsibilities
- Collaborating with the Interdisciplinary Treatment Team to develop and maintain individualized treatment plans
- Providing clinical oversight for residents and managing chronic and acute medical conditions
- Co-managing resident care in coordination with attending physicians and other providers
- Conducting regular assessments, progress reviews, and care plan updates
- Completing timely and accurate clinical documentation in the Computerized Patient Record System (CPRS)
- Participating in GEC and medical staff meetings for performance improvement initiatives
- Leading and participating in care transitions, ensuring proper communication for transfers
- Responding to medical emergencies and urgent clinical needs during regular hours
- Ensuring residents and families are informed of changes in condition or treatment plans
- Supporting facility performance measures and clinical standards
- Maintaining required professional credentials and adhering to safety, infection control, and ethical care standards
- Demonstrating professionalism and strong interpersonal communication
- Reporting incidents of patient abuse or safety concerns to appropriate leadership

Requirements
Experience: Experience in geriatrics preferred; 2 years of geriatrics experience preferred.
Education: Post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
License & Certification:
- Active, current, full, and unrestricted APN license
- Basic Board Certification in relevant Nurse Practitioner area
