Job Responsibilities
- Report directly to the Administrator, Executive Director or Patient Care Manager Senior.
- Function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor.
- Help the patient to maintain good personal hygiene.
- Assist in maintaining a safe and healthy environment.
- Prepare and serve light meals.
- Assist in feeding patient when needed.
- Assist the patient in ambulation.
- Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.
- Assist with light housekeeping tasks such as emptying trash and changing linens.
- Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations.
- Attempt to promote patient's mental alertness through involvement in activities of interest.
- Give basic emotional and psychological support to the patient and other members of the household.
- Report any change in the patient's mental or physical condition or home situation to the Registered Nurse and members of the interdisciplinary team.
- Demonstrate basic care to the primary caregiver.
- Practice infection control measures and standard precautions.
- Provide accurate documentation with visit itineraries.
- Attend staff meetings.
- Participate in at least one Quality Improvement activity yearly, as assigned by supervisor.
- Complete documentation in a timely manner as outlined in orientation.
Required Skills
Core Skills
- Certified Nursing Assistant (CNA) license
- Compassionate, kind and empathetic attitude
- Ability to provide care in various settings like home
- Flexibility and composure in crisis situations
Preferred Skills
- Six months of patient care experience in hospice, home health, ICU, geriatric, or oncology
- Experience in hospital settings such as RN, Med Surg, PACU, Float pool, ER, critical care, etc.
- Current CPR certification
- Current driver's license and automobile insurance
