Job Responsibilities
- Provide clinical coverage for the primary APC during periods of PTO or short/long term absences.
- Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions.
- Manage members during a short term, sub-acute rehabilitation episode.
- Conduct comprehensive, monthly/routine, and/or acute care visit assessments.
- Effectively manage acute and chronic medical and behavioral conditions.
- Ensure accurate and complete documentation, including ICD 10 conditions.
- Communicate and collaborate with the interdisciplinary care team.
- Conduct advanced illness and advanced care planning conversations.
- Provide patients and caregivers with counseling and education.
Required Skills
Core Skills
- Active and unrestricted license in the state of Florida
- Certified Nurse Practitioner or Physician Assistant
- Graduate of an accredited master’s degree in Nursing (for NPs)
- Current, active DEA licensure/prescriptive authority
- 1+ years of hands-on post grad experience in a long-term care setting
- Ability to complete physical job requirements including lifting a 30-pound bag and navigating various dwelling conditions
- Driver's License and access to reliable transportation
Preferred Skills
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional skills
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including ability to document medical information
Requirements
Experience: 1+ years of hands-on post grad experience (long-term care setting preferred)
Education: Graduate of an accredited master’s degree in Nursing (MSN) program for NPs; Graduate of an accredited Physician Assistant degree program for PAs
License & Certification:
- Active and unrestricted license in Florida
- Certified Nurse Practitioner or Physician Assistant through a national board
- Current, active DEA licensure/prescriptive authority
Schedule:
- No on-call, no weekends, and no holidays required
