Job Responsibilities

  • Assist the primary provider in gathering patient information for the initial health history of new patients.
  • Conduct follow-up phone calls on behalf of the care team provider.
  • Contact patients to discuss test results, complete referrals, changes in medications or care plan, and follow-up on self-management goals.
  • Serve as primary contact/clinical liaison between referral hospitals, pharmacies, treatment facilities, and patients/families.
  • Triage patients on the team's panel call or walk in and assist or direct them as appropriate.
  • Facilitate, schedule, and participate in various patient care coordination activities.
  • Maintain and update the team's patient panels.
  • Identify, create, and maintain panels of high-risk sub-populations for care management.
  • Collect and report clinical measures for a panel of patients.
  • Lead a multi-disciplinary health care team in performance improvement efforts.
  • Provide emergency care as needed to patients.
  • Assess and provide support in health promotion and disease prevention to patients with common chronic illnesses.
  • Initiate and follow up on patient needs for continuing care or related health services.
  • Provide patient and family counseling, including telephone triage and counseling with proper documentation.
  • Conduct nurse visits per standing orders and current orders in the patient's record.
  • Check stock of drugs, wound care supplies, linens, medical supplies, and other patient-related supplies.
  • Work closely with other members of the health care team in developing patient care visit plans.
  • Maintain patient records by completing thorough and accurate documentation of all patient encounters.
  • Administer medications as prescribed by the primary physician or provider.
  • Lead/coordinate pre-visit planning for scheduled patients.
  • Assist in the clinical area by performing treatments and procedures.
  • Participate in Quality Improvement teams and other committees as assigned.
  • Act in the absence of the supervisor.
  • Perform other job duties as assigned.

Required Skills

  • Knowledge of Tohono O'odham culture, customs, and traditions
  • Knowledge of HIPAA-related rules and regulations
  • Knowledge of established nursing concepts, principles, and practices
  • Knowledge of pharmacology of various drugs
  • Ability to conduct patient education classes
  • Skill in providing care and reacting to emergencies
  • Ability to provide individual and family counseling
  • Skill to develop Patient Visit Plans
  • Ability to maintain privileged, confidential information
  • Computer skills to utilize and document in the Electronic Health Record

Preferred Skills

  • Fluency in Tohono O'odham language and English
  • Experience in ambulatory care or clinic setting

Requirements

Experience: One (1) year of experience in ambulatory care or clinic setting is preferred but not required

Education: Degree from a professional nursing program approved by the legally designated State-accrediting agency

License & Certification:

  • Active, current, and unrestricted license as a professional nurse
  • Current certification in Cardio Pulmonary Resuscitation (CPR)
  • Current certification in Automated External Defibrillator (AED)
  • Current immunization statement including proof of a non-reactive tuberculosis skin test and documentation of immunity for hepatitis B
