Job Responsibilities

Collaborates with the Nemours Value Based Care Organization to ensure compliance with credentialing standards.
Screens patients in the acute inpatient care environment for gaps in care and opportunities for care coordination.
Collaborates with providers, case managers, social workers, and related care teams to understand care and treatment goals.
Uses clinical expertise to assess patient progression towards anticipated outcomes.
Identifies patients at risk for poor transitions, high Emergency Department utilization, and/or readmission.
Completes comprehensive transition assessments and plans for ongoing contact with at-risk patients.
Communicates needs and plans to providers and care teams.
Addresses barriers to achievement of patient care goals.
Makes appropriate referrals to care coordinators and care managers.
Ensures established care plans are dynamic and address all settings throughout the continuum of care.
Completes necessary interventions for transitions to home or skilled nursing facility.
Makes referrals to home care, DME, and pharmacy vendors.
Provides after-visit summaries and details related to ongoing care.
Works with other Nurse Case Managers on anticipatory discharge needs for short stay patients.

Preferred Skills

Case Management Certification: CCM or ACM-RN
Experience in pediatric care

Requirements

Experience: Five or more years

Education: BSN required

License & Certification:

Active Delaware or compact RN licensure
Case Management Certification preferred
