Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial, re-assessment, and focused assessment based on patient condition, setting, and population.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care and assures continuity of care during hospitalization.
Documents nursing care in the patient's record.
Administers medications and provides ordered treatments.
Educates patients and families on care procedures.
Continuously evaluates both the patient’s status and the effectiveness of the nursing care.
Modifies care plans as needed.
Coordinates care with patients, families, and/or members of the healthcare team.
