Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carry out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic assessments including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Identifies problems based on clinical judgment regarding patient’s health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan.
Documents care in the patient's record.
Ensures patient safety through effective medication administration and provision of ordered treatments.
Provides patient and family education.
Continuously evaluates the patient’s status and nursing care effectiveness, modifying the care plan as needed.
Coordinates patient care from admission to discharge.
Delegates and supervises responsibilities.

Experience:
