Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessment, re-assessment, and focused assessment based on patient condition, setting, and population.
- Identifies problems and sets measurable and achievable goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care.
- Implements nursing care according to the care plan and ensures continuity of care.
- Documents care in the patient's record.
- Ensures patient safety through medication administration and provision of ordered treatments.
- Educates patient/family about nursing care.
- Continuously evaluates the patient's status and the effectiveness of nursing care.
- Coordinates care with patients, families, and healthcare team members from admission to discharge.
- Delegates and supervises nursing tasks.
- Prioritizes patient care.
