Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment and re-assessment based on patient condition.
Identifies problems based on the patient's response to actual or potential health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care based on assessment data.
Implements nursing care according to the care plan.
Documents care in the patient's record.
Ensures patient safety through medication administration and provision of ordered treatments.
Educates patient and family.
Continuously evaluates the patient’s status and the effectiveness of nursing care.
Manages care coordination from admission to discharge.

Experience: 1 year of experience as a licensed RN

Education: Graduation from an accredited School of Nursing
