Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment of the patient.
Performs re-assessment and focused assessment based on patient condition, setting, and population.
Identifies problems based on the patient's response to actual or potential health conditions or needs.
Plans and sets measurable and achievable short- and long-range goals collaboratively with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care and ensures continuity of care during hospitalization and in preparation for discharge.
Administers medications and provides ordered treatments.
Educates patients and families.
Documents nursing care in the healthcare record.
Continuously evaluates both the patient's status and the effectiveness of nursing care.
Manages and corrects the sequence, timing, and effectiveness of patient care from admission to discharge.
