Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment, re-assessment, and focused assessment based on patient condition, setting, and population.
Identifies problems related to the patients' health conditions or needs.
Plans measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care based on assessment and problem identification.
Implements nursing care according to the care plan, ensuring continuity of care.
Documents care provided in the patients' record.
Evaluates both patients' status and the effectiveness of nursing care continuously.
Coordinates care actions with patients, families, and healthcare team members.
Delegates and supervises nursing tasks, prioritizing patient care.
