Job Title: RN Discharge Planner 2025-1896

Position: Registered Nurse in Discharge Planning

Job Type: Full Time

Pay:

Shift: Compressed Work Schedule: 9 hour shifts Monday-Thursday, 0600-1800; 8 hour shift one Friday per pay period, 0600-1800 with alternate Friday off

Job Responsibilities

  • Develops and executes discharge planning activities for the MTF.
  • Follows all approved discharge planning Clinical Decision Support Tool (CDST) criteria.
  • Ensures collaborative communication processes between primary care and medical management teams.
  • Collaborates with Health Care Integrator, medical management staff, and primary/specialty care teams for population health strategies.
  • Coordinates and participates in interdisciplinary team meetings and Care Coordination meetings.
  • Shares knowledge and experiences relevant to nursing, discharge planning, and case management.
  • Attends meetings during regular duty hours and professional staffing services.
  • Participates in the orientation and training of other staff.
  • May serve on committees, workgroups, and task-force at the facility.
  • Completes required training and competency verification programs.
  • Maintains productivity and quality with minimal supervisory direction.
  • Completes medical record documentation and coding.
  • Documents a comprehensive discharge plan in the electronic health record.
  • Conducts standardized peer review or gap analysis as directed.

Requirements

Experience: Experience in nursing and discharge planning preferred

Education: Associates Degree of Nursing

License & Certification:

  • Basic Life Support (BLS)
  • Advanced Cardiac Life Support (ACLS)
  • Pediatric Advanced Life Support (PALS)

Schedule:

  • Compressed work schedule with specified hours
