Position: Registered Nurse Case Manager for Home Health
Job Type: Full Time
Pay: $45.00 - $63.00 per visit/unit; $70,500 - $96,900 per year

Job Responsibilities
- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
- Confer with physician to develop the initial plan of treatment based on physician's orders and initial patient assessment.
- Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders under Clinical Manager's supervision.
- Revise the care plan in consultation with physician based on ongoing assessments as required by policy/regulation.
- Coordinate appropriate care with various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
- Report patient care, condition, and progress to the physician and Clinical Manager continuously.
- Implement patient care plan with the patient and family to assist them in achieving optimal resolution of needs/problems.
- Coordinate, oversee, and supervise work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plan that reflects the current plan of care.
- Monitor documentation completion by home health aides/personal care workers as part of supervisory/leadership responsibility.
- Discharge patients after consultation with the physician and Clinical Manager, preparing and completing necessary clinical documentation.
- Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
- Prepare visit/shift reports, summarize patient records, and confer with other health care disciplines to provide optimum patient care.
Required Skills
Core Skills
- Diploma, Associate, or Bachelor Degree in Nursing
- A minimum of one year of nursing experience preferred
- Strong med surg, ICU, ER, acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- A valid driver’s license, auto insurance, and reliable transportation
Preferred Skills
- Home Health experience
Requirements
Experience: A minimum of one year of nursing experience preferred
Education: Diploma, Associate, or Bachelor Degree in Nursing
License & Certification:
- Registered Nurse licensure
- CPR certification
