Job Responsibilities

Implement and monitor patient care plans.
Monitor, record, and communicate patient condition as appropriate.
Serve as the primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
Conduct initial assessments and re-assessments based on patient condition.
Identify problems and make clinical judgments about the patient’s responses to health conditions or needs.
Plan measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establish daily goals with the patient and family and formulate a plan of care.
Implement nursing care according to the care plan and ensure continuity of care.
Document care provided in the patient’s record.
Evaluate both the patient’s status and the effectiveness of nursing care continuously.
Coordinate actions with patients, families, and healthcare team members to manage patient care.
Delegate and supervise tasks as necessary.

Experience:

Education: Graduation from an accredited School of Nursing

License & Certification:

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) - American Heart Association
