Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessments, re-assessments, and focused assessments based on patient condition, setting, and population.
- Identifies problems based on clinical judgment about the patient’s health conditions or needs.
- Plans and sets measurable, achievable short- and long-term goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care and documents it in the patient’s care plan.
- Implements nursing care according to the care plan to ensure continuity of care.
- Administers medications and provides ordered treatments.
- Educates patients and families regarding nursing care.
- Documents care in healthcare record.
- Continuously evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
- Coordinates care actions with patients, families, and/or healthcare team members from admission to discharge.
- Delegates tasks and supervises other staff as required.
- Prioritizes nursing interventions based on patient needs.

Requirements
Experience: None required
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) - American Heart Association
