Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessment, re-assessment, and focused assessments based on patient condition, setting, and population.
- Identifies problems based on clinical judgment about the patient’s response to actual or potential health conditions.
- Establishes measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Forms a plan of care based on assessment data and problem identification.
- Implements nursing care according to the care plan, ensuring continuity of care.
- Ensures patient safety through medication administration and provision of ordered treatments.
- Educates patients and families regarding care.
- Documents care in the healthcare record.
- Continuously evaluates patient’s status and effectiveness of nursing care, modifying care plans as needed.
- Coordinates care actions with patients, families, and healthcare team members.

Education: Graduation from an accredited School of Nursing

- Basic Life Support (BLS) Certification - American Heart Association
