Job Responsibilities

Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
Identifies problems based on patient responses to health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care and documents it in the patient's care plan.
Implements nursing care according to the care plan to ensure continuity of care during hospitalization and preparation for discharge.
Ensures patient safety.
Administers medications and provides ordered treatments.
Educates patients and families.
Documents care in the healthcare record.
Continuously evaluates both the patient's status and the effectiveness of the nursing care, modifying the care plan as needed.
Coordinates actions with patients, families, and/or members of their healthcare team.
Delegates, supervises, and prioritizes nursing care.
