Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial, re-assessment, and focused assessment based on patient condition.
Identifies problems and sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care documented in the patient's care plan.
Implements nursing care according to the care plan to ensure continuity of care.
Documents care in the patient's record.
Evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
Coordinates actions with patients, families, and healthcare team members.
Delegates, supervises, and prioritizes nursing tasks.
