Job Responsibilities
- Assess, diagnose, and manage chronic and acute conditions
- Provide health education
- Develop treatment plans and effectively communicate them to residents and other providers
- Order and interpret appropriate lab and diagnostic testing and adjust treatment plans accordingly
- Maintain compliant documentation within EMR software
- Collaborate with operations and clinical leadership to achieve goals
- Support strategic initiatives regarding value-based care
- Provide excellent customer experience to foster high satisfaction/retention
- Practice ethically in accordance with professional standards and board certification
- Follow state and federal regulations, guidelines, and laws
- Collaborate with the interdisciplinary care team at the facility
- Coordinate care with other providers and telemedicine specialists
- Complete required regulatory visits and acute visits as needed
- Participate in family conferences and provide advanced care planning and education
- Address pharmacy recommendations or communications regarding residents
- Ensure adequate face-to-face coverage at assigned facilities during work hours

- Training or clinical experience in geriatrics
- Experience with coding, ICD-10 and HCC

- Training or experience in geriatric, assisted living, and/or skilled nursing settings
- Training or experience in outpatient primary care settings
Requirements
Experience: 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
Education: Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
License & Certification:
- National certification from AANP or ANCC
- Active, unrestricted state license
- Active DEA license
Schedule:
- Must be willing to work 5 days a week, Monday - Friday
