Collaborating with the Interdisciplinary Treatment Team to develop and maintain individualized treatment plans
Providing clinical oversight for residents and managing chronic and acute medical conditions
Co-managing resident care in coordination with attending physicians and other providers
Conducting regular assessments, progress reviews, and care plan updates
Completing timely and accurate clinical documentation in the Computerized Patient Record System (CPRS)
Participating in GEC and medical staff meetings for performance improvement initiatives
Leading and participating in care transitions, ensuring proper communication for transfers
Responding to medical emergencies and urgent clinical needs during regular hours
Ensuring residents and families are informed of changes in condition or treatment plans
Supporting facility performance measures and clinical standards
Maintaining required professional credentials and adhering to safety, infection control, and ethical care standards
Demonstrating professionalism and strong interpersonal communication
Reporting incidents of patient abuse or safety concerns to appropriate leadership

Experience: Experience in geriatrics preferred; 2 years of geriatrics experience preferred.

Education: Post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
