Job Title: Nurse Practitioner (CLC)

Position: Key clinical leader and member of the Interdisciplinary Treatment Team

Job Type: Full Time

Pay: Competitive salary, regular salary increases, potential for performance awards

Job Responsibilities

  • Collaborating with the Interdisciplinary Treatment Team to develop and maintain individualized treatment plans
  • Providing clinical oversight for residents and managing chronic and acute medical conditions
  • Co-managing resident care in coordination with attending physicians and other providers
  • Conducting regular assessments, progress reviews, and care plan updates
  • Completing timely and accurate clinical documentation in the Computerized Patient Record System (CPRS)
  • Participating in GEC and medical staff meetings for performance improvement initiatives
  • Leading and participating in care transitions, ensuring proper communication for transfers
  • Responding to medical emergencies and urgent clinical needs during regular hours
  • Ensuring residents and families are informed of changes in condition or treatment plans
  • Supporting facility performance measures and clinical standards
  • Maintaining required professional credentials and adhering to safety, infection control, and ethical care standards
  • Demonstrating professionalism and strong interpersonal communication
  • Reporting incidents of patient abuse or safety concerns to appropriate leadership

Requirements

Experience: Experience in geriatrics preferred; 2 years of geriatrics experience preferred.

Education: Post-master's certificate or master's or doctoral degree as a Nurse Practitioner.

License & Certification:

  • Active, current, full, and unrestricted APN license
  • Basic Board Certification in relevant Nurse Practitioner area
