Job Responsibilities
- Provide care for patients with acute and chronic illnesses encountered in the older adult patient.
- Take full accountability for patient care and outcomes, seeking consultation from specialists as needed.
- Engage with the hospitalist whenever a patient is in the hospital.
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation.
- Lead the care team consisting of care promoter, care facilitator, and care coordinator for office patients.
- Engage with the transitional care team for patients unable to come to the office.
- Play an active role in managing the center and cover for other providers as needed.
- Recruit patients for the center and additional providers for the company.
- Supervise and collaborate with Advanced Practice Practitioners while remaining accountable for their actions.
- Perform other duties as assigned and modified at manager’s discretion.

Requirements
Experience:
Education: MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
License & Certification:
- State Medical License
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred
- Current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification within first 90 days of employment
