Collaborate with patients and primary care providers to ensure quality, efficient, and cost-effective healthcare services.
Triage incoming patient calls and assess needs, giving appropriate clinical dispositions and facilitating referrals.
Educate patients regarding immediate care advice and preventative behaviors.
Perform professional assessments of callers' needs using established criteria and protocols.
Document all conversations with patients to maintain a comprehensive medical record.
Facilitate referrals within clinics by scheduling and coordinating appointments.
Support patients in the selection of the best physician and facility to maximize access and manage healthcare costs.
Provide health education and coach consumers on treatment alternatives.
Identify gaps in care and offer opportunities for intervention.
Communicate with Case Management, Community Health Workers, patients, families, and physicians/providers for safe transitions of care.
Conduct Clinical performance improvement and quality activities.
Use data to drive decisions and performance improvement strategies for patients.
Continuously evaluate laboratory results, diagnostic tests, and utilization patterns.
Work with leadership to evaluate processes and propose improvement strategies.
Educate multidisciplinary teams about clinical pathways and managed care principles.

Experience: Three (3) years of nursing experience

Education: Current unencumbered licensure as a registered professional nurse or multi-state licensure through the eNLC
