Job Responsibilities
- Provide care coordination for hospice patients throughout the Midlands East area.
- Service patients in their homes or Assisted Living Facilities.
- Communicate, collaborate, and coordinate patient care.
- Conduct continuous review and revision of the CCM plan of care.
- Facilitate telehealth visits with nurse practitioners for care coordination.
- Coordinate with the hospice interdisciplinary team and other individuals involved in the patient's care plan.
- Participate in hospice IDG meetings.
- Rounding with physicians in assisted living or skilled nursing facilities.
- Assess and chart observations of the patient’s condition.
- Collaborate with healthcare teams to meet patient quality measures.
- Ensure follow-up visits are scheduled.
- Participate in quality improvement projects.
- Maintain an updated list of providers and specialists in the EMR.
- Utilize company software systems for updates and communication.
- Perform other duties as requested or required.
Required Skills
Core Skills
- Licensed nurse with good standing
- Clinical experience (minimum of 3 years preferred)
- Effective supervision and direction of personnel
- Strong written and verbal communication skills
- Basic computer knowledge
- Leadership skills
- Good interpersonal skills
