Job Responsibilities

Maintains current knowledge of the local, state, and Federal regulations and laws which apply and assures compliance.
Completes an initial, comprehensive and ongoing comprehensive assessment of Senior/client and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous illness(es).
Assesses and evaluates Senior’s/client’s status by writing and initiating plan of care that establishes goals.
Regularly re-evaluates Senior/client and family/caregiver needs.
Participates in revising the plan of care as necessary, including the Senior/client and the family in the planning process.
Uses health assessment data to determine nursing diagnosis.
Initiates appropriate preventative and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Provides health care instructions to the Senior/client as appropriate per assessment and plan.
Assists the Senior/client with activities of daily living and facilitates efforts toward self-sufficiency.
Acts as Case Manager when assigned by Patient Care Coordinator.
Completes, maintains and submits accurate and relevant clinical notes.
Communicates with the physician regarding the Senior’s/client’s needs and reports changes in condition.
Teaches the Senior/client and family/caregiver self-care techniques as appropriate.
Works in concert with the interdisciplinary team.
Assists in providing continuity of care.
Takes on call responsibilities as assigned.
Drives from one Senior/client location to another.

Required Skills

Core Skills

Registered Nurse License
Knowledge of hospice care
Nursing assessment skills
Ability to communicate effectively with Seniors/clients and families
Patient management and care coordination

Preferred Skills

Experience in community health and/or hospice
Bachelor’s degree in Nursing

Requirements

Experience: Two years of acute care/long term care nursing experience.

Education: Successful completion of a Registered Nurse course of study; Bachelor's degree in Nursing preferred.

License & Certification:

RN License in good standing
CPR certification
Valid Nebraska Driver’s license
