Job Responsibilities
- Maintain a high performing hospice program.
- Manage, oversee, and direct patient care activities to ensure quality patient care delivery.
- Teach, evaluate, and develop associates.
- Ensure adherence to company policies and procedures, and compliance with local, state, and federal regulations.
- Oversee and manage rescheduled, declined, missed, and reassigned visit requests.
- Make decisions regarding priority and staffing based on clinical judgment.
- Respond to phone referrals/inquiries in a timely, professional, and compassionate manner.
- Triage medical concerns within scope of practice.
- Facilitate and prepare for the Interdisciplinary Group (IDG) meetings.
- Preside over IDG and present patients for review.
- Coordinate minutes and review patient charts to address any changes in patient condition or plan of care.
- Solicit input from various care team members.
Required Skills

- Direct patient care experience
- Knowledge of hospice principles and practices

- Ability to advocate for hospice patients and their families
Preferred Skills
- Bachelor's degree in nursing
- Experience in home-based care setting
Requirements
Experience: At least three years of direct patient care and two years in a hospice or home-based care setting.
Education: Graduate of accredited school of nursing; Bachelor's degree in nursing preferred.
License & Certification:
- Current Registered Nurse licensure in state of residence
