Job Responsibilities

  • Report directly to the Administrator, Executive Director or Patient Care Manager Senior.
  • Function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor.
  • Help the patient to maintain good personal hygiene.
  • Assist in maintaining a safe and healthy environment.
  • Prepare and serve light meals.
  • Assist in feeding patient when needed.
  • Assist the patient in ambulation.
  • Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.
  • Assist with light housekeeping tasks such as emptying trash and changing linens.
  • Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations.
  • Attempt to promote patient's mental alertness through involvement in activities of interest.
  • Give basic emotional and psychological support to the patient and other members of the household.
  • Report any change in the patient's mental or physical condition or home situation to the Registered Nurse and members of the interdisciplinary team.
  • Demonstrate basic care to the primary caregiver.
  • Practice infection control measures and standard precautions.
  • Provide accurate documentation with visit itineraries.
  • Attend staff meetings.
  • Participate in at least one Quality Improvement activity yearly, as assigned by supervisor.
  • Complete documentation in a timely manner as outlined in orientation.

Required Skills

Core Skills

  • Certified Nursing Assistant (CNA) license
  • Compassionate, kind and empathetic approach to patients and their families
  • Flexibility and composure while supporting others in a time of crisis

Preferred Skills

  • Six (6) months of patient care experience in hospice, home health, ICU, geriatric, or oncology
  • Experience in hospital settings such as RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home or travel
  • Current driver's license and automobile insurance
  • Current CPR certification

Requirements

Experience: Six (6) months of patient care experience preferred

Education:

License & Certification:

  • Certified Nursing Assistant (CNA) license
  • Current CPR certification
