Job Responsibilities
- Identifying patient/family/facility care needs
- Providing nursing and supportive care for terminally ill patients and their families
- Completing initial assessments
- Preparing and implementing care plans
- Providing direct nursing care
- Supervising Home Health Aide (HHA) care delivery
- Cooperating with Clinical Manager and interdisciplinary team
- Engaging in problem-oriented charting
- Teaching family and facility staff patient care skills
- Ensuring equipment availability
- Attending to patients and families at time of death
- Documenting in facility chart
- Maintaining relationships with facility staff
- Reconciling medications
- Updating visit schedules
- Attending care plan meetings
- Providing educational in-services
- Participating in marketing and customer service activities
