Obtain history and physical on new patients
Order/interpret diagnostic tests
Diagnose and treat chronic conditions and episodic illness
Perform Advance Care Planning
Accurate and timely documentation in the EHR
Coordinate with other members of the care team, both internal and external partners
Conduct annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines
Review and discuss the need to close care gaps and quality measures
Perform/review standardized screening exams
Identify all existing medical conditions
Accurately document assessment and conditions including quality MEAT Statements
Identify patients in need of chronic care management
Identify patients in need of pharmacy assistance
Document Elements of Personal Prevention Plan and health issues to follow up with PCP
Ensure a safe and smooth transition of patients following an acute or skilled stay or ED visit
In-home post-acute visit including history and physical and medication reconciliation
Manage short-term episodic illness related to acute or skilled admission
Coordinate care with team members for safe discharge planning
Identify barriers/obstacles to safe, self-management in the community and work with care team

Two years experience in specialty care area (Geriatrics)
Experience in private physician office, clinic setting or community-based program

Experience: Minimum two years experience in specialty care area (Geriatrics)
