Pay: Hourly range $19.86 to $38.85 based on full-time employment

Shift: Monday-Friday, 9:30am - 6:00pm

Conduct a high volume of outbound calls to assist members, evaluate current health status, discuss non-compliance to medication(s), schedule pharmacy telephonic appointments, and/or obtain important follow-up information from providers.
Answer inbound calls from members and assist them with their inquiries.
Identify patients with missing measures and work to close the measures through education/counseling, appointment setting, and other means as appropriate.
Follow system scripting and validate member demographic information.
Document the provider or member's record with accurate information obtained on the call.
Maintain education/knowledge base of HEDIS/STARs standards and guidelines.

Ability to multitask and perform all other related duties as assigned.

Current, unrestricted LVN/LPN license
3+ years of clinical experience as a LVN or LPN
3+ years of experience in the health industry and/or in a large complex matrixed organization
Intermediate level of proficiency with MS Excel, Word, and PowerPoint
Intermediate level of proficiency with SharePoint

Experience: 3+ years of clinical experience as a LVN or LPN; 3+ years of experience in the health industry
