Job Title: Clinical Quality LPN - Remote in PST
Position: Patient Navigator
Job Type: Full Time
Pay: $19.86 to $38.85 per hour
Shift: Monday-Friday, 9:30am - 6:00pm (No weekends, no holidays)

Job Responsibilities
- Conduct a high volume of outbound calls to assist members and evaluate current health status.
- Discuss non-compliance to medication(s), schedule pharmacy telephonic appointments, and/or obtain important follow up information from providers.
- Answer inbound calls from members and assist them with their inquiries.
- Identify patients with the missing measures and work to close the measures through education/counseling, appointment setting, and other means as appropriate.
- Follow system scripting and validate member demographic information.
- Document the provider or member's record with accurate information obtained on the call.
- Maintain education/knowledge base of HEDIS/STARs standards and guidelines.
- Uphold UHG Cultural Values.
- Ability to multitask and perform all other related duties as assigned.
Required Skills
Core Skills
- Current, unrestricted LVN/LPN license
- 3+ years of clinical experience as a LVN or LPN
- 3+ years of experience in the health industry and/or in a large complex matrixed organization
- Intermediate level of proficiency with MS Excel, Word, and PowerPoint
- Intermediate level of proficiency with SharePoint
Preferred Skills
- Experience in a call center
- HEDIS/STAR experience or participation with similar regulatory reporting
Requirements
Experience: 3+ years in health industry and clinical experience as LVN/LPN

- Current, unrestricted LVN/LPN license
