Job Responsibilities
- Plans for targeted patient populations.
- Performs resource management including denial management and utilization management.
- Conducts access to appropriate levels of care, discharge planning, care facilitation, and referral to other levels of care.
- Collaborates with the multidisciplinary care team to facilitate achievement of desired treatment outcomes.
- Manages all aspects of transition/discharge planning for assigned patients.
- Monitors the patient's progress and intervenes as necessary.
- Provides education to staff, physicians, and patients and their families.
- Meets directly with the patient and/or family to assess needs and develop an individualized transition/discharge plan.
- Initiates and facilitates referrals to post-acute services.
- Communicates necessary information regarding transition/discharge plan to the multidisciplinary team, patient, and family.
- Documents interactions with patients and all transition/discharge planning activities.
- Assists patients/families with completion of medical power of attorney and advanced directives.
- Collaborates for appropriate resource and financial management.
- Utilizes quality screens in the electronic record to identify potential issues.
- Completes clinical reviews for patients and applies approved utilization criteria.
- Educates hospital staff and physicians on payer regulations and managed care principles.
Required Skills
Core Skills
- Current unencumbered license with the WV Board of Registered Professional Nurses or appropriate state board.
- Clinical experience in nursing.
- Knowledge of patient’s medical insurance coverage and limitations.
- Knowledge of procedures and techniques for administering treatments.
Preferred Skills
- Bachelor’s degree in Nursing (BSN).
Requirements
Experience: Three (3) years clinical experience.
Education: Current unencumbered licensure as a registered professional nurse.
