Job Responsibilities
- Manages all aspects of transition/discharge planning for assigned patients in a timely manner.
- Collaborates with members of the multidisciplinary team to facilitate the transition/discharge process for designated caseload.
- Monitors the patient’s progress and intervenes to ensure a patient-focused, high-quality, efficient, and cost-effective plan of care.
- Provides education to staff, physicians, patients, and their families on effective transition planning.
- Assesses needs of the patient and/or family to develop an individualized transition/discharge plan in collaboration with the physician team.
- Communicates with the multidisciplinary team and post-acute providers about complex family dynamics impacting patient care.
- Initiates and facilitates referrals to post-acute services including Homecare, Durable Medical Equipment, Hospice Care, Long Term Acute Care Facilities, Acute Rehab Facilities, and Skilled Nursing Facilities.
- Communicates transition/discharge plan information to the multidisciplinary team, patient, and family.
- Provides timely documentation of interactions and planning activities according to departmental policy.
- Assists patients/families with completion of medical power of attorney, health care surrogate, and advanced directives.
- Collaborates on resource and financial management, including financial assistance coordination/referrals.
- Utilizes quality screens in the electronic record to identify issues like avoidable delays and readmissions.
- Completes clinical reviews for patients and documents findings based on department standards.
- Facilitates covered day reimbursement certification and engages in discussions with clinical staff regarding payor criteria.
- Educates hospital staff and physicians on payer regulations and managed care principles to prevent denials.
- Fosters staff and/or students' integration into the healthcare team.
Required Skills
Core Skills
- Current unencumbered licensure as a Registered Professional Nurse.
- Three years of clinical experience.
- Knowledge of patient insurance coverage and precertification requirements.
- Knowledge of nursing procedures and techniques.
Preferred Skills
- Bachelor's degree in Nursing (BSN).
Requirements
Experience: Three (3) years clinical experience.
Education: Current licensure as a Registered Professional Nurse.
