Job Responsibilities
- Supervises daily operations of discharge planning and social services in the inpatient and emergency department setting
- Provides direct supervision of Care Management staff
- Acts as the primary point of contact for inpatient unit Care Management related barriers/concerns
- Manages own assignment and patient/department outcomes of those under direct supervision
- Assists Care Management Manager/Facility Director to implement Corporate Care Management strategies
- Ensures active staff participation in daily Multidisciplinary Rounds
- Serves as a point of contact for all identified stakeholders
- Acts as a content resource regarding medical necessity criteria, patient status, and discharge criteria
- Assures staff documentation is completed per guidelines and coaches staff as needed
- Facilitates care conferences for complex transitions and/or placements, as needed
- Participates in hospital/medical staff meetings to review patients meeting criteria related to high dollar/extended length of stay or difficult placement
- Reports pertinent quality/risk issues to appropriate individuals, departments, and/or committees
- Discusses patient, staff, physician, and other department complaints with Care Management Manager/Facility Director and provides a written report within 24 hours if needed
- Demonstrates a positive problem-solving attitude to staff, physicians, and peers
- Serves as a supportive change agent for Care Management model processes
- Acts as a role model for Care Management department staff
- Encourages team member engagement
- Maintains open communication with staff
- Assists with staff performance reviews/evaluations
- Provides support and oversight with progressive corrective action plans
- Balances schedules to meet department needs prior to posting
- Communicates changes to expected schedule and minimizes unscheduled absences
- Plans job responsibilities for maximum utilization of staff
- Resolves daily staffing issues
Required Skills
Core Skills
- Registered Nurse: Associates Degree Nursing or Social Worker: BSW
- Two years of experience in direct Care Management
- State specific RN license if a nurse
Preferred Skills
- Registered Nurse: BSN or Social Worker: MSW
- Three years of experience in direct Care Management
- CCM, ACM, or RN-BC in Care Management
Requirements
Experience: Two years in direct Care Management
Education: Associates Degree in Nursing or BSW
License & Certification:
- State specific RN license if a nurse
