Job Responsibilities
- Collaborate as an active member of the interprofessional team to provide patient and family centered care.
- Collect and assess data related to patient health status to develop, execute, and evaluate the plan of care.
- Utilize evidence to drive clinical decisions and practice related to individual and population health.
- Establish goals and strategies for meeting care coordination needs with patients, families, and/or care providers.
- Plan and deliver care considering factors related to safety, effectiveness, and fiscal accountability.
- Delegate and supervise care in accordance with the WI Nurse Practice.
- Participate in continuous quality improvement initiatives.
- Assume responsibility for professional development.
- Coordinate patient care and provide direct patient care.
- Educate patients and perform/assist with in-clinic minor procedures.
- Perform telephone triage/eVisits.
- Float to other areas as needed for operations.
Required Skills
Core Skills
- Clinical experience in patient care and management
- Ability to collect and assess patient health data
- Knowledge of evidence-based clinical practice
- Patient care coordination skills
Preferred Skills
- At least 1 year of relevant nursing experience
- Bachelor's Degree in Nursing
Requirements
Experience: At least 1 year of relevant nursing experience, preferred.
Education: Nursing Licensure Required. Bachelor's Degree in Nursing, Preferred.
License & Certification:
- Current state of Wisconsin Registered Nurse License
- Multi-state Nursing License from a participating state in the NLC
- AHA Basic Life Support for the Healthcare Provider (BLS) required within 90 days of hire
Schedule:
- Tuesday 8:00am-4:30pm
- Wednesday 8:00am-4:30pm
- Friday 8:00am-4:30pm
