Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments and re-assessments.
Performs focused assessments based on patient condition, setting, and population.
Identifies problems through clinical judgment about the patient’s response to actual or potential health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care and assists in transition or discharge planning.
Implements nursing care according to the care plan to ensure continuity of care.
Documents care in the patient’s record.
Ensures patient safety and medication administration.
Provides ordered treatments, patient, and family education.
Continuously evaluates both the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
Coordinates care with patients, families, and healthcare team members.
Delegates, supervises, and prioritizes nursing care.
