Job Responsibilities

Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments.
Performs re-assessments.
Conducts focused assessments based on patient condition, setting, and population.
Identifies problems based on clinical judgment about the patient’s response to actual or potential health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan to ensure continuity of care.
Documents patient care in the patient’s record.
Ensures patient safety.
Administers medications.
Provides ordered treatments.
Educates patients and families.
Continuously evaluates the patient's status and the effectiveness of nursing care.
Manages care coordination with patients, families, and/or members of the healthcare team.
Delegates, supervises, and prioritizes nursing tasks.

Required Skills

Core Skills

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) certification

Preferred Skills

Advanced Cardiac Life Support (ACLS) certification
Pediatric Advanced Life Support (PALS) certification
Neonatal Resuscitation Program (NRP) certification

Requirements

Experience: None

Education: Graduation from an accredited School of Nursing

License & Certification:

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) certification
Advanced Cardiac Life Support (ACLS) certification (preferred)
Pediatric Advanced Life Support (PALS) certification (preferred)
Neonatal Resuscitation Program (NRP) certification (preferred)
